Photo Release Form

I hereby grant and authorize RadiFi Credit Union the right to take, edit, alter, copy, reproduce, exhibit, publish, distribute and make use of any and all pictures or video taken of me and my likeness, and that of my minor child, to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment, royalty, or any other consideration.

This authorization extends to all languages, media, formats, and markets now known or later discovered. This authorization shall continue indefinitely unless and until I notify RadiFi Credit Union Attention: Marketing Department, in writing of the withdrawal of my consent. I waive the right to inspect or approve any finished product in which my likeness appears, including written and electronic copy. I understand and agree that any materials provided to the Credit Union and any pictures or video of me and my likeness, and that of my minor child, shall become the property of RadiFi Credit Union and will not be returned. I hereby hold harmless and release RadiFi Credit Union its employees, officers, directors, successor, assigns, and the photographer from all liability, petitions, claims, and causes of action which I, my heirs, representative, executors, administrators, minor child, or any other persons may make while acting on my behalf or on behalf of me or my estate including, but not limited to, any claims for defamation, libel, slander, or invasion of privacy.

I have read the foregoing, fully understand and consent to the contents hereof, and agree on behalf of myself and any minor child.
